Volunteer Waiver
Please read carefully before participating

This page provides example language for a volunteer information and waiver document. It is for informational and draft purposes only and does not constitute legal advice. You may wish to have your own attorney review and customize this language before use.

1. Nature of the Product
Cosmetic-style cream, not a drug

Montana Flower Arnica Recovery Cream is a topical cosmetic-style product designed to support the appearance and comfort of bruised or stressed skin. It is not a prescription medication and is not intended to diagnose, treat, cure, or prevent any disease or medical condition.

No claims are being made regarding medical outcomes. Any improvements you may notice relate to cosmetic appearance or personal comfort only.

2. Voluntary Participation
You may stop at any time

Your participation as a volunteer is completely voluntary. You may choose to stop using the product and withdraw from the evaluation at any time, for any reason, without penalty.

If you decide to withdraw, you simply stop using the cream and, if requested, let the organizer know so your feedback can be recorded appropriately.

3. Eligibility & Health Considerations
When to avoid participation

You should not participate if:

  • You are allergic or sensitive to arnica or any listed ingredient in the cream.
  • You have open wounds, broken skin, or active skin infections in the area where you would apply the product.
  • You have been advised by a healthcare provider to avoid topical products of this type.

If you are unsure whether this product is appropriate for you, consult your healthcare provider before participating. Volunteers are encouraged to perform a small patch test on an unaffected area of skin prior to more widespread use.

4. Possible Risks & Discomforts
Common-sense precautions

As with any topical product, there is a possibility of skin reactions, including redness, itching, burning, rash, or other irritation. If you experience any discomfort or adverse reaction:

  • Stop using the cream immediately.
  • Gently wash the area with mild soap and water.
  • Contact your healthcare provider if symptoms persist or worsen.

Serious reactions are not expected but are theoretically possible in sensitive individuals. In case of any concerning symptoms, seek medical care promptly.

5. Use Instructions
How volunteers will be asked to apply the cream

You will receive written instructions with your jar of Montana Flower Arnica Recovery Cream. In general, volunteers are asked to:

  • Apply a thin layer to intact skin over or around bruised areas, as directed.
  • Avoid contact with eyes, mouth, and mucous membranes.
  • Wash hands after application.

Detailed instructions, including frequency and duration of use, may be adjusted as the product evolves.

6. Feedback & Photos
How information may be collected

Volunteers may be invited to share:

  • Written feedback about comfort, texture, scent, and overall experience.
  • Optional photos taken before and after a period of use, focused on bruised areas.

Your name and direct identifying details will not be published without your explicit permission. Photos may be cropped or de-identified if used for internal review or future marketing, and only with your consent.

7. Acknowledgment & Release
Draft example language

By checking the waiver box on the Volunteer form and choosing to use the product, you acknowledge that you:

  • Have read and understood the information on this page.
  • Understand that Montana Flower Arnica Recovery Cream is not a drug and is not a substitute for medical care.
  • Voluntarily assume the ordinary risks associated with using a topical cosmetic-style product.

You agree to discontinue use and seek appropriate care if you experience any adverse reaction, and you release the organizer from liability for minor, expected risks consistent with typical cosmetic use, to the extent permitted by applicable law.

This sample wording is provided for convenience and should be reviewed and customized with your own legal advisor if used for an actual project.
